Estimated date the site is expected to go live: ____________________________, 20________.
Include an explanation of your relationship to the proposed site (owned, leased, operated by other party, etc.)
If any of the site registration information submitted is incomplete, it could result in a delay in the site approval process. Additionally, you should include any other pertinent information that will be helpful in the site selection process in a cover letter to the Selection Committee. Once all the site paperwork has been received, the Selection Committee will review the proposed site. You will be notified of their decision in writing. Upon approval, a CFN site number will be assigned. No site approval or equipment authorization will be given without being reviewed and approved by the Selection Committee.
It should be noted that each site approval is valid for 120 days. Within that time frame, the site is expected to go live on the network. If there are any delays, please notify the Site Transition Coordinator in writing by mail or by fax by or before the 120th day with a brief progress report which must specify how much additional time is needed. OTHERWISE, AFTER 120 DAYS YOUR APPROVAL WILL BECOME INVALID. If your site registration becomes invalid or is cancelled, it will be necessary for you to reapply and repeat the site approval process. If you repeat the approval process, you will need to submit an additional check for $300.00.
This site registration packet includes all the necessary forms required for review and approval of your site by the Selection Committee. If you have any questions, or would like to obtain new site registration packets, please do not hesitate to call the Site Transition Coordinator at (650) 356-3028.
