Application and Review Process of CFN Premier Sites

New Sites – Participant must complete the Premier Site Status Request Form and return it to CFN in the Site Request Package.  You can obtain a form on www.cfnnet.com under Membership, Site Registration.

Questions regarding Premier Site designation, whether qualified or non-qualified, should be directed to the Site Transition Coordinator for review by the Selection Committee.
